A Pre-Assessment Interview will be conducted by a member of our team. If you do not receive a call or missed a call, please call us and ask for a member of our intake team.
Once you have completed the Pre-Assessment Interview, please complete each of the following forms from the first section, then all of the forms under the section appropriate for your insurance:
Private Pay, Commercial and Medicaid insurances ONLY:
Supports for Community Living (SCL) Waiver or Michelle P Waiver (MPW) ONLY:
Once we have received all of the forms, they will be reviewed and we will contact you to schedule and in-person Needs Assessment.
After the needs assessment, our Intake team will meet to review your information and data collected to determine the needs of the individual and assign a BCBA. If we cannot accept the individual, you will be notified at this time.
The assigned BCBA will write a Behavior Support Plan (BSP) and or Functional Analysis (FA) to guide the care of the individual.
The BSP will then be sent to the individual's insurance provider with a Prior Authorization Request. The time required to process this request varies by insurance.
Once the Prior Authorization is approved, we will call you to set up the participant's regular schedule and services will begin.
